Patient Safety: Deborah Schoch of the CHCF Center for Health Reporting examines the complexities of deaths linked to hospital infections in California and why these deaths remain "in the shadows."

Chronic Fatigue: New doubts have been raised about a scientific study that linked chronic fatigue syndrome to a retrovirus called XMRV, and editors of the journal that published the 2009 study have asked its authors to voluntarily retract the paper, Amy Dockser Marcus reports for The Wall St. Journal.

Medicaid: In California, about 380,000 elderly or disabled people on the state's Medicaid program, known as Medi-Cal, will be moved into managed care plans in a bid to ease the state's budget crisis, Victoria Colliver reports for the San Francisco Chronicle. Advocates worry about disrupted care during the transition and possible denials of medical care by health plans.

Food Safety: A former Wendy's employee is an unlikely food safety hero, posting this YouTube video of black mold next to hamburger burns, dirty stove and other unsanitary conditions at a Wendy's location in New York.
